The Active System Nursing Assessments

All Nursing Assessments are patterned after the MDS and the answer push directly to the resident's current MDS. Weekly and Monthly Summaries are available.

Nursing Instruction Sheets are Available for each residnet which can be posted at the bedside and/or available as a reference on the screen or in CNA notebooks.

Setup Individualized Instruction sheets and then make them available everywhere. On the screen of any computer, and at the Bed Side, and in Notebooks and with Care Plans and transmit or Email them anywhere.


ADL, Hydration and Turning Schedule sheets are available and easy fill out. Completion Reports can be run in seconds to deteremine what has not been recorded.....

The Active System provides you and your staff with facilitated input of a variety of common forms. These forms are presented on the screen as they appear on the printed page. Everyone relates to them immediately and can fill them out quickly.


Once the Staff is filling out their forms on the Active System, you will have an invaluable auditing tool. You will be able to instantly determine who has documented their work and who has not. Train your charge Nurses to run these reports on the screen and/or by print out before the end of each shift.

The X's in the above report indicate that more than 9 holes occur in that particular day's ADL sheet for that particular resident. This is an actual report. Don't let this happen in your facility. Are you sure it is not happening already ?


Meal Consumption sheets can be part of the ADL Sheet, or a seperate sheet as shown in Figure 8. This form offers more emphasis and instruction to your staff

Meal Consumption is becoming one of the main records of resident care. You need to be absolutely sure that they are being filled out and that they are accurate. The best way to do this is to have them filled out on the Active System and then make the information available to ALL staff .


CNA Assignment Sheets Allow You to Assign and Keep Track of Who Does What, Who Did it When and When it Was or Was NOT Done.

Executive Reports align=

The Active System has an Employee Schedule program, but this does not tell you who cared for which resident. The CNA Assignment Sheets will tell you this and will allow you to back track to any occurance or incident.


The Resident Album helps you to orient New CNA's and Nurses to their assignments.

With a Digitial Camera ($200) and the Active System Put Picture Software, it's very easy to download pictures to the system and assign them to individual residents. In fact, you can take ALL the pictures for your resdents in just a 2-3 hours for a 120 bed Nursing home.

Record I/O's faster and add them up accurately. This will also ensure that I/O's can be tracked and alerts will occur if I/O's are not completed, or not consistent with Physician's orders.
